By using our medical services at Hernandez Medical PLLC, you agree to be bound by these terms of service. Please read these terms carefully before receiving care at our facility.

Patient Requirements

As a patient, you agree to:

• Provide accurate and complete medical history information
• Arrive on time for scheduled appointments
• Give 24 hours advance notice for appointment cancellations
• Follow prescribed treatment plans and recommendations
• Treat staff and other patients with respect and courtesy

Appointment Policies

Our appointment policies include:

• Appointments are scheduled during regular business hours
• Same-day appointments may be available based on provider availability
• Cancellations require 24 hours advance notice
• Late arrivals may result in appointment rescheduling
• No-show appointments may be subject to a fee

Payment and Billing

Payment terms and billing policies:

• Payment is due at the time of service
• We accept cash, check, and major credit cards
• Insurance verification is required prior to appointments
• Co-pays and deductibles are due at time of service
• Outstanding balances may be subject to collection procedures

Insurance and Coverage

Regarding insurance coverage:

• We participate with most major insurance plans
• Insurance verification is the patient's responsibility
• We will file claims on your behalf when possible
• You are responsible for services not covered by insurance
• Prior authorizations must be obtained when required

Medical Records

Medical records policies:

• Medical records are available upon written request
• A processing fee may apply for record copies
• Records will be released only with proper authorization
• Electronic records may be available through our patient portal
• Records are retained for a minimum of 7 years

Emergency Care

For medical emergencies:

• Call 911 or go to the nearest emergency room
• Our office provides urgent care during business hours
• After-hours emergencies should be directed to emergency services
• We are not responsible for emergency care provided by other facilities

Limitation of Liability

Our liability is limited to the medical services provided within the scope of our practice. We are not responsible for treatment provided by other healthcare providers or facilities.

Modifications to Terms

We reserve the right to modify these terms of service at any time. Changes will be effective immediately upon posting. Continued use of our services constitutes acceptance of modified terms.
